Required Forms & Paperwork
Please complete all steps below. Forms must be complete, correct, and signed no later than one week prior to the start of your program; however, please note that some forms, particularly health-related ones, may be due even earlier.

First things first
Pay your remaining program balance
You must submit any remaining program balance by May 30, 2025. You may lose your place if you do not meet your payment deadline.
International students
As an international student, you must obtain an F-1 student visa to attend the Precollege Summer Program. It can take weeks to process your request, so leave yourself as much time as possible.
Activate your NetID and Cornell email address
Your NetID is your key to secure online services at Cornell. Activate it along with setting up your Cornell email account (with security questions) as soon as you receive an email with instructions from Cornell Information Technologies (CIT). CIT will begin sending activation emails in late March. You'll receive your CIT email about a week after you've been admitted to the program.
If you have any questions about NetIDs or need help activating your NetID or setting up your Cornell email address, please contact the IT Support Desk during business hours.
Enroll in Cornell's emergency mass notification system
To ensure you receive critical information and alerts, after activating your NetID, visit the Student Center. Under the "Personal Information" category, enter your contact information under the links for "Emergency Contact Info" and "Emergency Mass Notification."
Submit special dietary requirements if needed
To have special meals prepared, please submit this form at least a week before your program begins.
Prepackaged kosher and halal meals can be ordered in advance through the dining supervisor or door checker on duty. Vegetarian and vegan options are available at all meals. Questions? Contact Cornell Dining.
Submit important forms
You will not be officially enrolled in your class(es) until all your required forms are complete.
Again, please be sure to also check your Accepted Student or Parent/Guardian Portal for any additional forms.
1. Health Information Forms
To ensure your well-being during your time at Cornell, you need to complete and submit two sets of medical forms:
- Cornell Health requires you to complete “New Student Health Requirements” as soon as possible. Note that failure to meet these requirements by June 12 will result being disenrolled. This information is required to meet state and federal laws, as well as university policy, and in the event you need to go to urgent care. If you have any questions, contact the Cornell Health Requirements Office at 1.607.255.4364.
- Summer Discovery* requires forms for the in-program nurse who will provide immediate, day-to-day care for your student during the program.
You will receive an email invitation from Playground (prepforsummer.com) to complete this and other required Summer Discovery forms.

* Cornell University’s School of Continuing Education has contracted with a third-party organization, Summer Discovery, to support components of our Summer 2025 Residential Precollege Studies Program, including the provision of residential staff, student activities, and health and safety management. Cornell University retains full control over all academic elements of the Summer 2025 Precollege Studies Program, including academic curriculum, instructor selection, and course content.
2. Student Health Plan (SHP) Enrollment Form
- Students living outside the U.S.
This coverage is required for students from outside the U.S. You'll automatically be enrolled in SHP. No action is needed on your part. - Students living within the U.S.
This coverage is optional for U.S. students taking six or more credits. If you do not have health insurance that meets Cornell's minimum requirements you may enroll in SHP. - For information, see Health services.
3. Disability Self-Disclosure and Request for Access—Accommodations Form
- Required only for students requesting disability services
- Deadline: As soon as you receive your NetID
- Submit this form (available after March 1) if you have a disability and need disability services to participate fully in the Precollege Summer Program. Advance notice is needed to arrange the timely implementation of accommodations, thus it is best to submit the Disability Self Disclosure and Request for Access Accommodations and your supporting documentation at least three weeks before you arrive to campus.

4. Photo Submission Form (NetID required)
- Required
- Deadline: As soon as your NetID is activated.
- You must upload your photo to receive your student ID card. This photo will be displayed on your ID card, in your student center, and on your professor's class roster. You must have an ID card to access campus facilities, use a meal plan, check out library materials, and more.
- You will receive your ID card during check-in on Opening Day.

View your class(es)
Your course(s) will appear in Student Center shortly after you receive confirmation of your enrollment. (Note: If you check your course in the Student Center, you’ll notice there’s a “hold” on your account. This simply indicates that if you want to make a course change, you must contact the Precollege Studies office.)
