Submit additional required forms
- Health Information Forms
To ensure your well-being during your time at Cornell, you need to complete and submit two sets of medical forms:
- Cornell Health requires you to complete “New Student Health Requirements” as soon as possible. Note that failure to meet these requirements will result in disenrollment from your program. If you have any questions, contact the Cornell Health Requirements Office at 1.607.255.4364.
- Summer Discovery* requires forms for the in-program nurse who will provide immediate, day-to-day care for your student during the program. You will receive an email invitation from Playground (prepforsummer.com) to complete this and other required Summer Discovery forms.
For Playground account questions, email cornellprecollege@summerdiscovery.com
* Cornell University’s School of Continuing Education has contracted with a third-party organization, Summer Discovery, to support components of our Summer 2026 Residential Precollege Studies Program, including the provision of residential staff, student activities, and health and safety management.
