Note: All forms must be completed, correct, and signed no later than one week prior to the start of your program; however, some forms particularly health-related ones, may be due even earlier.

Be sure to check for additional items on your Accepted Student or Parent/Guardian Portal.

Please contact us if you have any questions.

International Students

As an international student, you must obtain an F-1 student visa to attend the Precollege Summer Program. It can take weeks to process your request, so leave yourself as much time as possible.

Submit additional required forms

  1. Health Information Forms

To ensure your well-being during your time at Cornell, you need to complete and submit Cornell Health medical forms:

    • Cornell Health requires you to complete “New Student Health Requirements” as soon as possible. Note that failure to meet these requirements will result in disenrollment from your program. This information is required to meet state and federal laws, as well as university policy, and in the event you need to go to urgent care. If you have any questions, contact the Cornell Health Requirements Office at 1.607.255.4364.
  2. Student Health Plan (SHP) Enrollment Form
    • Students living outside the U.S.
This coverage is required for students from outside the U.S. You'll automatically be enrolled in SHP. No action is needed on your part.
    • Students living within the U.S.
This coverage is optional for U.S. students taking six or more credits. If you do not have health insurance that meets Cornell's minimum requirements, you may enroll in SHP.
    • For information, see Health services.

Orientations

Students and their parents/guardians should be on the lookout for an email invitation for our on-campus orientation, during which Precollege staff will share important information and host an optional campus tour.

  • For students in Three-week Session 2 and the Six-week Session: Saturday, June 20, 2026 | Time and location TBD
  • For students in Three-week Session 3: Saturday, July 12, 2026 | Time and location TBD

Also, during the program, students will be invited to a lunch with Precollege staff to discuss academics, ask questions, and connect with other commuter students.
