All campers must submit the following forms and waivers through the secure, online platform CareMonkey.com. Access to CareMonkey will be available after you have submitted your deposit.
This required information must be completed in English and submitted two weeks prior to camp or your child will not be able to participate in the program.
- Medical Authorization Form
New York State law requires a licensed prescriber's signature for any medication to be dispensed by our staff. A "medication" is any substance taken to maintain and/or improve health and includes over-the-counter drugs, vitamins, natural remedies, and first-aid cream.
- Immunization Form
New York State law requires all campers submit a physician's vaccination record of the immunizations they have received since birth. In addition, New York State has experienced a measles outbreak in several counties. New regulations require that your camper have up-to-date immunizations. Unfortunately, there are no exceptions to this policy.
- Media Consent Form
This form gives you the opportunity to indicate whether or not you grant Cornell permission to record or photograph your child and use that likeness, image, and/or voice in promotional, informational, or other materials.
- Travel Release Form
This form allows Cornell CAU staff to transport your child in designated vehicles to various locations on and off campus for planned activities.
- Participant Waiver
This form is required for campers participating in activities, such as sailing or climbing, where additional risk is assumed by the participant.
- Permission Form for Teen Check In/Out
This form allows teens to check themselves in and out of the teen program each day.
- Permission Form for Residential Teen Arrival/Departure
This form allows residential teens to sign themselves in at the start of the week when the program begins and sign themselves out at the end of the week when the program ends.
